- Present passport and previous passports in original
- Photocopy of present passport (front and back page)
- Appointment Page
- DS-160 Confirmation Page
- Present Passport must be valid for 6 months and have 2 blank pages
- Two recent passport photographs (50 x 50 mm) with white background
- Covering letter on company letterhead stating name, designation, purpose and duration of visit and grant of leave
- Detailed tour itinerary
- Proof of confirmed accommodation and return tickets
- Proof of Finance
(A) Bank statement for last 6 months
(B) ITR for last 3 years
(C) Original Bank account statement for last 6 months. Has to be printed on banks stationery, signed and stamped by the bank on each page.

(A) Brief company profile
(B) Proof of ownership (Partnership deed, articles of association or proof of proprietorship)
(C) ITR of Company
(D) Bank statement for last 6 months of company

(A) Copy of appointment letter
