Documents Required For Ukraine Visa From India
- Completely filled application form
- One recent passport-size photograph (white background)
- Passport
(A) Must have validity of 3 months after intended period of travel
(B) Have at least 2 blank pages left.
- Overseas medical insurance with minimum coverage of Euro 30000. (photocopy)
- Complete travel itinerary with return ticket/reservation
- Photocopy of Applicants passport
- If visiting family/friends, standardized invitation issued by the regional body or state division of Immigration Service
- If diplomatic visit, invitation from the required ministry or other central authorities or organizations
- If visiting for medical treatment, an invitation letter from the medical treatment facility
- If visiting for business, a document confirming that the applicant is co-founder of the venture/company etc and is travelling to monitor the performance
- If visiting for religious purpose, an invitation from a registered organization attested by the Ministry of Culture
- Confirmation from the committee on Humanitarian Aid of the Cabinet of Ministers of Ukraine for religious matters
- Letter confirming that the nature of travel is tourism in accordance with the Ukrainian Law on Tourism
- Proof of Social/Professional Status
- Proof of Adequate Financial Means
(A) Salary slips/certificate last 6 months
(B) Bank Statement of 6 months
(C) ITR last 3 years
(D) Form 16

(A) Hotel Confirmation
(B) Letter of Invitation from Sponsor along with photo id proof.
